IPO Questionnaire and due diligence checklist
The purpose of this Questionnaire is to obtain certain information
to be used in the preparation of the Registration Statement, although
responses to this Questionaire will not necessarily provide all the
required information. It will be helpful to organize the responses
to the questions in a set of loose leaf notebooks indexed by number.
A copy of the responses to this questionnarire should also be given
to counsel for the underwriters in connection with their due diligence
review of the Company.
